The annual monitoring visit is complete and either there were no corrections, no Corrective Action Plan was needed, or all corrections to a corrective action plan have been made.

Annual Health and Safety Monitoring Visit Complete - Not in Compliance
The annual monitoring visit is complete. The provider did not fully correct all items on the Corrective Action Plan. The Child Care Resource and Referral Agency (CCR&R) has sent the 30-day closeout notice and this program is no longer eligible for CCAP. Please reach out to your local CCR&R for more information.

Health and Safety Corrective Action Plan in Progress
The annual monitoring visit was made and corrections are needed. A Corrective Action Plan has been put in place with the provider, and they are within the accepted timeframe to make the corrections.
This is a temporary designation that could last up to 90 days and either switches to “in compliance” or “not in compliance” based on the results.

The annual monitoring visit has not been held yet, but it is scheduled. Please reach out to your local CCR&R for more information.

Annual Health and Safety Monitoring Visit Not Completed this Year
The annual monitoring visit has not taken place this year because the provider may not be caring for kids any longer or the monitoring was deferred for a year. Please reach out to your local CCR&R for more information.
